USCHI HALL OF FAME NOMINATION FORM
1. Provide a detailed list of the candidate's special contributions
to harvesting. A biography or resume format may be used
for this purpose. Please include a summary of how this person
has touched people or communities through his or her involvement
in harvesting.
2. List awards earned by the candidate, offices held or
other honors received, along with year(s) award was given
or office held, if available.
3. Please include the following with your submission:
* A photo or engraving of the candidate, suitable
for enlargement.
* Letters of support from community members,
co-workers, family and friends.
* Additional photos, news clippings and anything
outlining the candidate's contribution to harvesting.
* Additional comments you may wish to make in
support of your nomination of this candidate.
Your submission may take the form of a folder, notebook
or scrapbook. However, any photos, clippings or other inclusions
must be firmly attached to secured sheets, rather than placed
loosely. Please place the photos you have selected for enlargement
in a seperate envelope or other protector, rather than attaching
it directly to any page. Secure its protector in the submission
to not damage the photo.
Candidates who are not selected for induction may be re-nominated
by their original nominators or others. The U.S. Custom
Harvesters, Inc. will not consider nominations more than
two years old without re-submission.
